Complete this form if you are a holder of a  current Victorian wildlife licence holder and you have experience theft or  escape of wildlife; or loss, theft or damage of Record Books or Return Forms.
Theft or escape of wildlife:
Holders of a Wildlife Licence have an obligation to notify  DEECA of any theft or loss of wildlife, including:
- Report any theft of wildlife to Victoria Police within two business days of becoming aware of the theft and retain a copy of the police report; and 
- Report any escape or theft of wildlife to DEECA within two business days of becoming aware of  the escape or theft; and  
- Record the incident in your Wildlife Record Book. 
Theft, loss or damage to Record Books and Return Forms:
- Holders of a Wildlife Licence must notify DEECA of the  theft, loss of or damage to any licence, record book or completed return form within two business days of being aware of the theft, loss or  damage. 
- If a Wildlife Licence or Wildlife Record Book has been stolen, lost or damaged, the  holder of the Wildlife Licence must pay a maximum fee of 1 fee unit for each  replacement licence and/or record book.
